Your Summary of Care Record is a concise document from your GP that includes key details about your health. It usually covers: 

• Your current and past diagnoses 
• Medications you’ve been prescribed 
• Allergies and adverse reactions 

This information helps your specialist clinic understand your medical background so they can assess whether medical cannabis may be appropriate for you. You generally need to have tried two conventional treatments before you are eligible for medical cannabis.

How Do You Request It? 

Requesting your Summary of Care Record is straightforward: 

• Contact your GP practice – You can call, email, or use your practice’s online portal if available. 
• Ask for a copy of your Summary of Care Record – Simply say you’d like a copy; you don’t need to explain why. 
• Receive your record – Many practices can send it electronically within a few days, while others may provide a printed copy. 

Do You Need to Explain Your Reasons? 

No. You have a right to access your medical records under UK law. GP staff may ask if you want it printed or emailed, but they won’t ask you to justify your request. 

How Long Does It Take? 

It varies slightly depending on the practice. It can take a week to receive but most can provide the Summary of Care Record within a few working days. Some may even issue it the same day.
